Pokračování brutální stealth akce Manhunt. V hlavní roli vědec Dr. Daniel Lamb z privátního blázince, ve kterém se pracuje na tajném programu vývoje zbraní založených na drogách. Lamb kooperující na projektu odtud však musí prchnout, přičemž na útěku spojuje své síly s psychotickým chovancem jménem Leo Kasper, jenž mu velice ochotně předvede kouzlo násilí a brutality. Podobně jako v prvním dílu budou dostupné tři různé způsoby likvidace nepřítele, z nichž ten první je hodně rychlý a ten poslední zase hodně násilný. Vedle přímého zabíjení vlastníma rukama či nástroji v těchto se počítá i s využitím okolního prostředí, jako když protivníka strčíte do otevřené rozvodné skříně, aby to dostalo potřebný náboj. Opět lze odlákávat, resp. přivolávat pozornost vyvoláním hluku.
